How do I make a new patient appointment?
Please contact us here. Alternatively, you may email info@brandperformancemed.com or call 425-230-3131. (Generally, anything sent over the internet, including email and contact forms are not private and may be potentially viewed by others. Any communication with Brand Performance Medicine may be recorded in your medical chart to assist in your care.) We look forward to speaking with you to set up your appointment!
What should I wear to my appointment?
You are welcome to bring comfortable/ athletic attire with ability to expose the area of concern. Often, however, it’s not necessary to change out of your normal clothes. Shorts are available if necessary.
How should I prepare for my first appointment?
  • Fill out your new patient paperwork (emailed to you upon registration)
  • Wear comfortable clothes
  • Bring your identification and insurance cards
  • Arrive 15 minutes early
  • Bring imaging, reports etc. that may be applicable to your injury
  • Workers Compensation and Motor Vehicle Crash patients- Please bring any reports, claim numbers, approved diagnoses or other information pertaining to your case.
How long is the initial appointment?
You should expect your initial appointment to last at least 120 minutes. This may involve a holistic history and physical examination, then making a short, medium and long-term plan in writing.
Where are you located?
Our address is 13333 Bel-Red Rd, #210, Bellevue, WA 98005. We are located across the street from Mox Boarding House, one block west of Wade’s Gun Shop.
Where should I park?
Our building has free parking. We are also conveniently located near bus stops along Bel-Red Road.
How do I know what my insurance will cover?

Brand Performance Medicine (BPM) expects payment for clinic visits at the time of service and does not directly bill your insurance company. The only exception is the Department of Labor & Industries (L&I) for state funded work related injuries, for which BPM will bill L&I as a courtesy on your behalf. BPM will provide you with a super-bill with the necessary codes to submit to your insurance company for reimbursement for services obtained from an out-of-network specialist provider. The amount of the coverage varies by insurance plan and we cannot guarantee payment from your insurer. We recommend you call the phone number of the back of your insurance card to find out your out-of-network specialist benefit in advance. Medicare and Medicaid do not cover our services. Our view is that insurance companies are first and foremost businesses whose primary objective is to control costs. We believe that goal does not always translate into doing what is best for your health. We are committed to doing what is best for our patients.

What is the average cost of the first visit?
  • $600/hour (minimum 2 hours). Full fee schedule available upon request.

Are there more affordable options?
  • Affordable phone, text, email and video consultations are an option.
  • Keep a doctor in your pocket: Phone, text and email telehealth follow-up consultations are billed at usual office rates in 10 minutes increments, an extra-ordinary value for timely access to a double-board certified sports medicine expert, often quicker and less expensive than an office visit.
  • Phone, text, email and video telehealth consultations offer the convenience of avoiding the commute, parking, missing work, etc., and can sometimes be provided out of state.
